Providing this information in advance helps speed up the insurance verification and billing process. All information collected is securely stored in your confidential medical record.
Non-Covered Insurance
Patients without medical insurance are expected to pay at the time services are rendered. We offer CareCredit and may arrange payment plans when appropriate.
All account balances are due upon receipt of the first billing statement. Credit is extended as a courtesy, and payment arrangements may be considered based on individual circumstances.
Full payment may be required before treatment if a patient has an overdue balance, a history of collections, or a recent bankruptcy.
Patients experiencing financial challenges may qualify for adjusted payment arrangements or financial assistance. A financial disclosure form and supporting documentation will be required for review.
Appointments that are missed or canceled without at least 24 hours’ notice may be subject to a missed-appointment fee. These charges are the patient’s responsibility.
Because insurance plans differ, we recommend contacting your insurance provider directly to confirm coverage and potential out-of-pocket costs. Your insurer may request treatment details such as diagnosis codes, CPT codes, or information related to durable medical equipment (such as braces). Our office is happy to provide this information upon request.
